Quantity:
Include a piece of paper with your name and the address to which you want the repaired QuadStick sent. A copy of the receipt from ordering this repair would be preferred.
Return only the QuadStick, with the lip position sensor, mouthpieces and the mounting ball attached to the back. Do not return mounting arms, super clamps, USB cables, CronusMax or other accessory parts, unless they are broken.
